Local 613’s CURRENT Health & Welfare Benefits Provided By The Union…Is This Enough Coverage?

Short Term Disability: $313 (before taxes) for 26 weeks. This is at least a 73% weekly income GAP.

Long Term Disability: $0 (ZERO) benefits after STD ends. No monthly income is a 100% income GAP.

Life: $4,000 AD&D burial benefit if death is cause by an accident.

About Open Enrollment

We are pleased to inform you that IBEW Local 613 will be in open enrollment beginning October 1st. All of the available coverages are offered separately and a member’s participation in one or all of these coverages, and the level to which they elect to participate, is voluntary and strictly their individual choice.

The purpose of this group insurance plan is to give you options to supplement your income and expenses in the event that you cannot work as a result of an injury or illness. The group plan offers Short Term Disability, Long Term Disability, and Life with Accidental Death & Dismemberment Insurance coverages through Sun Life Financial.

All insurance under this Union Group Policy will cover you as long as you continue to remain actively employed and in good standing with the union.

Group Insurance Information

This page will provide you with information related to your union’s group insurance plan. Here you can view and download a copy of the group policy/benefits booklet, view rates, and request information. If you have any questions regarding the benefits options that you are currently enrolled in, please call 847-387-3555.

Please note that if you are suspended, dismissed, or retire, you are no longer eligible for disability coverage and should contact our office immediately at 847-387-3555.

Disabilities that occur after being removed from service are not covered. If you do not contact our office within 30 days of your date of dismissal or date of retirement, there will be no refund for any premiums paid. It is the responsibility of the member to contact Cornerstone Benefits Management at 847-387-3555 or by email at info@uniondisability.com.

You will be able to resume your coverage if you return to work within 12 months. If dismissed or suspended, you may keep your life insurance coverage for up to 12 months as long as premiums are paid. Beyond 12 months, your coverage will terminate unless you convert or port the policy.

Reminder

This is a group insurance plan; therefore, the costs and benefits are determined by the overall claims experience of the group. Your future rates and benefits may change regardless of whether or not you’ve ever filed a claim.

Know Your Responsibilities

It is the responsibility of the member to notify Cornerstone Benefits Management if employment or union status changes. Failure to properly notify Cornerstone Benefits Management could result in loss of premiums and/or insurance coverage. Notification to Cornerstone Benefits Management must be by phone at 847-387-3555 and/or by email at info@uniondisability.com.
