File a Claim
Important!!! A member of our team can assist you if you need to file a claim. Before you call, please gather the following information:
- Doctor’s name, phone number and fax
- Proof of your last day of work and a 2 week pay stub
- Checking account and routing number for direct deposit benefit payments
